STEP 3: Download, Fill Out & Return the Wedding Information Sheet

              Please mail the completed form, with deposit (to secure your wedding date) to:

STEP 4: Download & peruse the Ceremony Options Guide

                   This is a unique system whereby you can design your own ceremony

                      by choosing from the options presented.

                   Once all of your selections are made, return to this website and fill out the

                      form below to create your custom-made ceremony!
